Youth Mental Health Transitional Day Program
Many vulnerable children and youth at Humber River Health require specialized, long-term care. The Youth Mental Health Transitional Day Program bridges this gap by helping young patients transition back to their homes, schools, and communities with vital therapeutic and academic support. This program, a partnership between Humber River Health and Lumenus Community Services, enables a more holistic approach to care.
Serving North West Toronto’s vibrant, multicultural community, we address mental health challenges compounded by systemic barriers and limited resources. For families here, transitioning from inpatient care to community life is complex, demanding a seamless, compassionate continuum of care beyond hospital walls.
Thanks to a generous gift from the Slaight Family Foundation, our Youth Mental Health Transitional Day Program was able to expand from 6-12 weeks, setting the stage for lasting, meaningful impact. This support has fueled recent program growth, allowing us to serve more youth in need. With new hires and a fully onboarded team, we’re poised to expand even further. Although early in its expansion, the program has built a strong foundation for transformative growth.
Our team reached full capacity at the end of August, with each new role undergoing a six-week training and onboarding process. Now operating at full strength, the program is prepared to scale quickly helping guide patients toward brighter, healthier futures. As we move forward, we aim to increase patient access by 65%, furthering our commitment to comprehensive mental health care in our community.
