Engaging with a dynamic and diverse community contending with mental health challenges

Focusing on those at risk of being underserved

TODAY/ CHALLENGE

There remains an urgent need to reach those groups who often fall through the cracks. These groups consist of transitional-age youth (17–25), youth experiencing their first psychotic episode, treatment-resistant populations, and individuals with severe mental health issues. Close-up, vignetted image of a family member holding hands with a patient. These groups deserve equitable access to comprehensive mental health care and a chance for recovery.

TOMORROW/ OPPORTUNITY

Your support will help to create continuity of care for youth with a Transitional Age Youth Program. It will also enable early intervention for first psychotic episodes through our Navigate model and advanced therapies like neurostimulation will give hope for those with treatment-resistant disorders. Additionally, your support will help establish the Assertive Community Treatment (ACT) team, providing intensive, personalized care for those with severe mental health issues. Your donation will help to fund intensive, integrated, and person-centred support to help individuals regain their stability.

To make these essential programs a reality, we need your support. Together, we can ensure that every member of our community receives the care they need and deserve.

Overcoming Barriers

Youths encounter numerous barriers when transitioning from adolescent to adult mental health programs. Over 50% of young people discontinue treatment during this critical period.

Early Intervention

Early intervention for a first psychotic episode is crucial. Psychosis typically begins in the late teen years or early adulthood, and without timely treatment, it can severely disrupt education, employment, and relationships.

Guiding Principles

Our “Navigate” model is designed to guide individuals through the mental health system and help them achieve personal goals through dedicated support from a committed treatment team.

Treatment Resistant

Advanced treatments, such as neurostimulation and ketamine-assisted therapies, hold immense promise for treatment-resistant individuals.

In the beginning there was a feeling of helplessness. Not knowing what to do, not knowing who to talk to. So being able to go to Humber and receive the care that we did. The consistent quality of care. We’re incredibly grateful that something like this exists.
